A normal level is: Direct (also called conjugated) bilirubin: less than 0.3 mg/dL (less than 5.1 µmol/L) Total bilirubin: 0.1 to 1.2 mg/dL (1.71 to 20.5 µmol/L) A high level of bilirubin in the blood is known as hyperbilirubinemia. High bilirubin levels can cause jaundice.Jaundice makes the skin and the whites of the eyes appear yellow, due to the brown If the unconjugated bilirubin levels are higher than the conjugated bilirubin, this could be caused by hemolytic or pernicious anemia, transfusion reactions, and cirrhosis. If the conjugated bilirubin levels are higher than the unconjugated bilirubin, this is typically due to a malfunction of the liver cells in conjugating the bilirubin. Some labs use different measurements or may test different samples. Conjugated or direct hyperbilirubinaemia occurs when the the liver is able to conjugate bilirubin, but the excretion is impaired. Causes include: failure of bilirubin excretion by hepatocytes: Dubin-Johnson syndrome; Rotor's syndrome; obstruction to biliary flow i.e. cholestasis, both intra-hepatic and extra-hepatic Suspected neonatal hyperbilirubinemia - conjugated hyperbilirubinemia is defined as a direct (conjugated) serum bilirubin level greater than 1 mg/dL if the total bilirubin is less than 5 mg/dL or as a direct bilirubin level more than 20% of total bilirubin if the total bilirubin level is greater than 5 mg/dL. If conjugated serum bilirubin is elevated, cholestasis is present and prompt stepwise clinical evaluation is necessary. Elevated conjugated bilirubin should lead to increased total bilirubin. If the patient has normal conjugated bilirubin levels and elevated indirect bilirubin levels, it is a sign either of excessive hemolysis or of a failure of the liver as it struggles to cope with the normal rate of hemolysis. The level of direct, indirect and total bilirubin will be measured as part of a standard liver panel test. Raised Bilirubin is the indicator of liver dysfunction. Bilirubin is the basic end product of Hemoglobin metabolism. Total Bilirubin (Direct Bilirubin + Indirect Bilirubin) Direct Bilirubin (Conjugated Bilirubin) Indirect Bilirubin (Unconjugated Bilirubin) Sample Required. This test is done in the serum of the patient. 3. Both conjugated and unconjugated bilirubin are elevated, values are almost the same, commonly results from hepatic diseases, for instance: Direct bilirubin 9.0 mg/dl, indirect bilirubin 8.0 mg/dl. Indirect bilirubin 12.0 mg/dl, direct bilirubin 11.5 mg/dl. Common bilirubin levels in infant newborns: Normal results for conjugated (direct) bilirubin should be less than 0.3 mg/dl.
